I'd put it down to luck. As much as we try to make the queue system fair and predictable, there will always be cases where posts are missed, looked at by different people with different opinions, mods will leave because of exams or holidays, etc. There's always forum #12247 for appeals.
Funny, I appealed an image that should've made it out of the mod queue and that was ignored too.
I gladly would have used my contrib status to upload it mnyself, and even that seems to have been overlooked ...
Only thing wrong was a few artifacts, and I've seen much, much worse in both art and artifacts approved lately.
I will say, for my part, if an image has untranslated text in it it gets judged on the basis of the image alone. If the text is translated, then I take the text into account. As many pictures that have text in them rely at least in some part on the text, this can make a significant difference, particularly with things like 4komas or doujinshi pages.
