What's the difference between shokuhou_misaki in toaru_kagaku_no_mental_out and in shokuhou_misaki -toaru_kagaku_no_mental_out? Is it something about her look, or just random tagging?

I think stuff tagged with Mental Out is explicitly stuff from the Mental Out manga, while everything else is much more generic.
For example, Misaki shows up a lot in New Testament / Genesis Testament and Railgun, so stuff from there wouldn't have the Mental Out tag.

"Impostor"...kinda? He did come from Touma / Imagine Breaker, so that situation is kinda blurry/complicated. I think Touma variant may be more accurate?

Even if it's taking Touma's form, it's a separate entity and isn't actually Touma. If only appearance was necessary to implicate a character to another, then you'd end up setting grounds that Misaka's clones should implicate Misaka or that Chloe von Einzbern should implicate Illyasviel von Einzbern.
