For hyphenation consistency with off-shoulder shirt, off-shoulder dress, off-shoulder sweater, etc.
Not equivalent tags. The off-shoulder_* tags are for clothes that are designed to be cut below the shoulders, while off_shoulder is for clothes that's left hanging off the shoulder despite not being designed for that purpose.
I used to get this wrong myself all the time and slap off_shoulder on posts that had one of the off-shoulder tags when the two are meant to be mutually exclusive

Then why do off-shoulder shirt, etc., imply off shoulder?
Let's not try to come up with fake reasons for a spelling mistake. It's obviously wrong, "off shoulder" is a typo, and it was arbitrarily picked in topic #4935 for no reason. If anything the proper name used in fashion is "off-the-shoulder". But then again it's a lot of post version churning for fixing it.
And yes, as brought up many times before the tag is for two different concepts, both for shoulder slips and actual fashion designs. They need to be separated, but nobody so far has come up with a good name to differentiate them. I think the last time we debated this we didn't have deprecation yet, so it might be worth revisiting that topic now.
