As far is I could tell the image wasn't uploaded already, so something affecting the md5 value must be the same? Is that how I should interpret that?

I think you clicked submit twice. So the second upload detected the first one as the duplicate. If you check the upload log there are two entries you made, two seconds apart, with the first one successful and the second one saying "Md5 has already been taken".
If it was actually an md5 collision you wouldn't have been able to upload the post.
No, now that I know what to look for, the message says its up already with the md5 note, it's detecting it as a duplicate and stopping a second upload attempt, which I interpreted as being up from earlier, but it was just the past 2 seconds. My mouse has been screwy and sometimes clicking weird, so Toks deduction is correct, it double clicked at upload time when it shouldn't have.

Wish there was a more efficient way to deter such bull-headed activity by one individual.
The ip recorded from the most recent account is the one that's banned, not a different one in the same range. Not sure how he got around it, maybe he used a different ip to create the account (bypassing the ip ban), then went back to his normal ip (so his alternate ip doesn't get recorded anywhere).
Short-term solution ... can we just lock down those pages from further editing?
Long-term solution ... some kind of new policy, like preventing new users from making changes for the first week? Another idea could be preventing multiple accounts from being opened up from the same IP address within a short-time period (like a month)?

I'm just throwing this idea out there, but why not extend "locking a wiki/pool/upload" function to all builder+ accounts. We're already given the ability to lock notes and ratings. This could even be a separate permission, like the "approve posts" permission, in case someone is found to be abusing this function. Since we're more numerous, we can be more responsive to vandalism like this, and take some of the administrative burden off of the moderators.
