I want to know the names of the small blonde and pink-haired chick with the black bow between Mugetsu and Shinki, the praying fairy in front of Kotohime and also who's the brunette in a green dress behind Akyu and Rinosuke.

So basically they're left nameless and are generically referred as just the "Makai Residents". I'll name them "Pixie" and "Demonica" instead (like how I name the jacket girl "Jackie" and the label girl "Labelle").
I have no idea too. No one of minor characters on Touhou wiki seems to have similar design.
My best guess is that she's likely the artist's original character (OC).

Maybe in a prequel, but I doubt they'd go past the TTG-era when Speed Symboli, someone who'd be Shinzan's kouhai, was explicitly referred to as an alumni in the L'Arc scenario.
