The viewer is being stabbed, has just been stabbed, or will imminently be stabbed. Usually, this is indicated by someone facing the viewerwielding and thrusting with a weapon going partially out of frame off the bottom or top border of the image. Artist's intent does not have to be an actual pov.
It's like aiming at viewer but with a knife, and the knife is often already in you.
