If the player refuses to be put down by Gehrman the First Hunter and defeats the old man in battle, the Moon Presence will come and incapacitate them. However, if along the way the player has consumed three specific items (One Third of Umbilical Cord), the Moon Presence will recoil, and instead challenge them to one final fight to end the Nightmare.
