Heart-shaped pupils if the pupil is heart-shaped. Heart in eye if there is a heart somewhere in the eye, not the pupil.

From what I can tell is that "heart_in_eye" implicates the common red heart being drawn inside of an eye. While "heart-shaped_pupils" commonly implies a heart shape being drawn inside of the eye.
Heart in eye is for images like post #8513048, where a heart shape is visible in the eyes but not the pupils (often used as a stylistic highlight). Heart shaped pupils is for images like post #8516703, where the pupils themselves are hearts (often used to indicate infatuation).
