Well, the downvoters didn't give any reason and the two tags look identical to me, plus one has less than 100 posts, so I'm approving this.

TBH I'd pick puffy anus as the main tag.

I didn't give any reason for my downvote because I thought that the differences between them are pretty clear if you just look at them properly. Puckered_anus is for the shape first and foremost, this tag requires anus to be pushed out and have round shape. Puffy_anuses can have any shape and may or may not be pushed out, the tag is for anuses that look more meaty than usual, analogous to puffy_nipples.

There is an overlap, but they have different purposes.
