I just noticed, that we have really alot of cameltoe and covered nipples cases, that are rated safe, but the howto:rate wiki states:

Rating:Questionable

Erect nipples under clothing.
Cameltoes and wedgies.

Should they still be rated questionable and what is the right indication for a cameltoe, that´s safe. I saw some images, where it was not that bad, like post #3795788, but I think if a cameltoe is visible, it should be rated q. There are also some mistagged images, but still alot of them have a clearly visible cameltoe or erect nipples under clothing. I´m rather asking before I change those images to questionable.

Yes, usually they should be rated questionable. That post, altough the cameltoe is slight, given the pose and her shirt i would deem it questionable, i don't think that's safe. It's a bit borderline i guess.

This actually raises a question: should there be a 4th rating category, for things between safe and questionable (doesn't show anything, but still not particularly safe for work)?

No.
People should just be aware of things that are considered "sexual".
I feel like Danbooru users, especially Builders that go for high score uploads, lack this sort of awareness. Or they want these images to be on safebooru so they reach more people (as said, score is "relevant").

In this case I´m going to fix the rating on pictures with a clearly visible cameltoes and erected nipples.

I'm not sure, but I think you could argue that blushing and breathing heavily might push the sexual implication enough outside of "normal human activity" to be explicit, particularly in the first image which displays "Blatantly exposed genitals".

From what I've seen it's being used for both what you describe as well as when it's unclear what the artist intends it to be but it looks like cum and/or pussy juice.
