Why eyebrow raise yet eyebrows raised? Shouldn't there be consistency?
I'd go with "raised" in all cases.

No I'm not. There's nothing under eyebrow raise or eyebrows raised so the aliases are presumably for ease of finding the tags. But why when doing this would you have an alias from eyebrow raise but eyebrows raised? It doesn't seem to make sense.

Yes, these are tags that make it easier to find raised_eyebrow / raised_eyebrows.
But I fail to see how you are not seeing that the 4th alias is for the latter.
It's simple grammar.
"Eyebrows [multiple] are [both/all] raised"

Likewise you could have
"Eyebrow [singular] is [one] raised"
