I was going off of what it was labeled where I found the photo, but it also looks like the photo has been floating around for at least ten years. If you can more positively identify it, please feel free to fix the tags.

EDIT: Digging deeper I think you're right. The only places calling it an Acty seem to be English. This Japanese site with a different view calls it a Carry: https://ameblo.jp/kobe-drivers-support/entry-12209330255.html

    I think that element is just a matter of age, old Honda Actys have round sealed headlights like this reference photo too, and newer Suzuki Carrys also all have molded rectangular headlights.

    From what I can see online the Carrys transitioned in the mid 1990's (95-97 or so) so this one must be older than that. The Actys already had rectangular headlights in 1990.

    Kinda. 4channers have been spreading the rrat (rumour/narrative) that Enna, Millie, and several other vtubers were the ones who bullied Dokibird. But this gossip is not backed up by anything and is most likely false. Just like several other rumours which appeared during that time and have already been debunked.

    "Oujou-sama" is a generic term for any young upper-class girl though. It's not like that couldn't be used as a qualified tag for the OC so long as the artist doesn't use the term for anyone else, but it's not a name per se.

    Precisely. I have seen other artists use generic terms for their original characters, but I was/am not confident to change the name without more information in this case
