It's not really a carrying item but an environment item you can interact with: if you know it the game becomes harder, and if you knock it again the game returns to normal level.

The wiki of Sekiro mentioned something called a Bell Demon, which is not the environmental bell that you ring, but something that you can use to return the game's difficulty to normal.
Since 鐘苦難 has the kanji of bell(鐘) come first, I was assuming this was what they were talking about.
Man... I really empathize with Admiral Hisamura here. I've tried a lot of chess but have never been able to learn any strategies other than "4-move mate" but put me in charge of Command: Modern Operations and I end up repeatedly beating Pyrpolitis 1-14 without any application of luck whatsoever.
