He is known under different names: 旧名は「山川良志信」「山川よしのぶ」「山川世忍」。 but the main one and used in recent anime is 山川吉樹 (Yamakawa Yoshiki).

According to Wikipedia やまかわ よしのぶ is the correct reading for 山川吉樹.

Atwiki also use Yoshinobu furigana. But ANN, MAL, AniDB use Yoshiki. Jisho romanizes 吉樹 only as Yoshiki and Yoshitatsu, though he can use a custom reading of his name. Also, AniDB has a separate Yoshinobu entry, which represents his early works.

On the Madhouse site, I found his personal page, which explicitly uses Yoshiki, and I'd say it's the most trusted info I found. And there seems to be no such page on jcstaff.co.jp

They do use yadama as an username, but looks like やだま is supposed to be romanized as ydm. From https://twitter.com/ydm_sushi:
