So Twitter have a option to load images in 4K, should I upload the normal version or the 4K version because idk which one would be a MD5 mismatch

What do you mean? Can you post an url example for the 4k version?
In any case, you should just use the tweet link on the upload page ( or on https://danbooru.donmai.us/uploads/batch for multi-image tweets), it'll automatically extract the right picture. No need to manually grab the direct image links.
Just for clarification, the best version of a twitter image has "orig" as final part of the url. Examples:

I am on desktop and do not have a "load 4K" option. See reblogger tweet mentioning this new iOS feature.

But yeah, like nonamethanks mentioned, to be sure, you should always just plug in :orig

It’s weird because when I open the image on Safari, it loses it 4K resolution, I think you can only see/save the 4K version on the Twitter app. The regular version also doesn’t have the orig, it have “large”

The "large" version is not the original, it's just a large enough picture to be viewable in browser. Make sure to not upload those by accident.
